Please review the following prior to booking:

Minors: If you are booking an intake session for a minor, please book the session under their name, and I will contact you with the appropriate consent forms for all guardians to fill out. Please note that all guardians must provide consent prior to me meeting with the minor.

For any minors (under 18), guardians are required to be present during the initial session for the purposes of reviewing the consent form and gathering information (history, therapy goals, presenting concerns, etc). It is strongly encouraged that all guardians attend, however if that is not possible, consent must be obtained from any guardians not present prior to me meeting with the minor. This can be arranged over the phone.

For minors under 12, I recommend that only guardians attend the initial session. For minors 12 and over, guardians are typically present for the first half of the initial session, and the remainder of the session is spent one on one with the minor and the psychologist. This can be flexible, depending on your needs.

Entering the Building: For security purposes, the main doors to the building are locked at all times. Please use the buzzer to get in – Code: 6047. This will alert me that you are outside. If I am in another session, I may not respond right away.
